This was the result of a problem with the database caused by SCORM (the part of Blackboard that lets things like Adobe Presenter or Articulate report quiz results back to the Blackboard Grade Centre).
To quickly fix the problem, Managed Hosting have disabled SCORM. It will be re-enabled after work to increase the capacity of Blackboard has been carried out.
